01 · DISCOVERY

How customers find you

People find mental health apps through App Store and Google Play search, recommendations from therapists and clinicians, employer benefit programs, and creators who talk openly about their own experience. Trust is the deciding factor: Mozilla's Privacy Not Included reviews flagged most mental health apps they examined in 2022, and the FTC's 2023 order against BetterHelp over sharing health data with advertisers showed what is at stake. Privacy and clinical credibility are marketing assets here, not footnotes.

The buying journey

Search in the store or hear a recommendation, scan screenshots, reviews and the privacy label, try a free exercise, then hit a subscription prompt. Many users try several apps; the one that helps in the first session and does not feel exploitative keeps them. Clinician-referred users arrive with higher intent and expectations.

What app makers aim for

  • Rank in store search for the needs you actually address
  • Earn recommendations from therapists and clinicians
  • Publish clinician-reviewed content that helps people today
  • Show verifiable privacy practices that set you apart

02 · CHANNELS

Channels, ranked by what matters most

Ranked 5 (essential) to 1 (optional) by the research for this industry.

  1. App Store and Google Play optimization with careful claims

    Essential

Store search is the main entry point. Apple reviews medical apps with greater scrutiny under 1.4.1 and expects healthcare-service apps to come from a legal entity; Google Play requires a Health apps declaration. Describe what the app does, not what it cures.

  2. Clinician and therapist channel

    High

Clinicians recommend tools for between-session practice. A clinician page with evidence summaries, a free professional account and printable handouts gives them a reason to recommend you.

  3. Clinician-reviewed skill content

    High

People search for help with a specific moment (grounding techniques, a thought record). Practical, reviewed articles that work without the app build trust and rank over months.

  4. Privacy as a visible feature

    Medium

Minimal tracking, no ad pixels on health screens and a readable privacy page are differentiators when reviewers and users compare apps.

  5. Creators with lived experience

    Medium

Creators who speak honestly about mental health reach people who trust them. Disclose partnerships clearly and never script health claims.

  6. Community participation

    Optional

Support communities are for support. Only participate where rules explicitly allow, never pitch in someone's crisis thread, and point to crisis resources rather than your app.

The same channel can be a strong fit for a paid game and a poor one for a free one. Each line below is a rule or rating from Outpost's knowledge base, with its source; your own plan is built from your answers.

Free with ads

  • WorksApple App Store

Apple rejects apps designed predominantly to display ads, limits display ads to the main app binary, requires interstitials to be labelled as ads with easy close/skip buttons and a way to report inappropriate ads, and bars third-party ads and analytics in Kids Category apps (except limited contextual ads from services that human-review creatives).

  • WorksGoogle Play

Google Play bans unexpected full-screen interstitials, full-screen interstitials that cannot be closed after 15 seconds (opt-in or non-interrupting ones, such as after a score screen, may run longer), ads that mimic app or system UI, and lock-screen monetization (unless the app is a lock screen); rewarded ads the user opts into are exempt.

Subscription

  • WorksApple App Store

Apple auto-renewable subscriptions must provide ongoing value, last at least seven days, work across all of the user's devices, and clearly describe what the user gets before asking them to subscribe.

Freemium

  • Strong fitAlternativeTo

AlternativeTo classifies apps as Free, Freemium (free version can 'perform the main purpose of the app'), Free for personal use or Commercial (time- or usage-limited versions count as Commercial), offers an 'Only Open Source' filter, rejects Coming Soon, closed/private beta, invite-only and Early Access apps, and will probably accept open betas.

Paid random items (gacha, loot boxes)

  • Apple App Store

On the App Store and Mac App Store, unlocking features, subscriptions, in-game currency, premium content or a full version must use Apple In-App Purchase, and apps may not use their own unlock mechanisms such as license keys.

  • Apple App StoreSome markets only

Since iOS 26.5 (terms published 18 June 2026), Brazilian iOS apps may use alternative in-app payments or link out to the web: App Store commission is 21% (10% for Small Business Program members and for subscriptions after year one) plus 5% if Apple processes the payment, link-out sales within 7 days carry a 15% (or 10%) store-services commission, and alternative marketplaces carry a 5% Core Technology Commission.

  • Google PlaySome markets only

Google Play user choice billing (an alternative billing system offered alongside Google Play billing, with a 4% service-fee reduction on those transactions) is available in India, and in Indonesia and Brazil under the pilot.

  • Apple App StoreSome markets only

Since Japan's Mobile Software Competition Act took effect in December 2025 (iOS 26.2), iOS apps in Japan can be distributed through Apple-authorised alternative marketplaces and can offer alternative payment processing or web purchase links alongside Apple IAP; App Store commission is 10% or 21% plus 5% for Apple IAP, web-link purchases pay 15% (10% for reduced programs), and apps outside the App Store pay a 5% Core Technology Commission.

Technical eligibility

  • Apple App Store

Apple moved to a new age-rating system; developers had to answer the updated age-rating questions by 2026-01-31 to avoid an interruption when submitting app updates.

  • Apple App Store

An App Clip's uncompressed, thinned main bundle may be at most 15 MB for iOS 16 and 10 MB below iOS 16; 100 MB is allowed only for iOS 17+, digital-invocation-only clips used where reliable internet is likely.

  • Apple App Store

App privacy details (the 'nutrition label'), including the data practices of third-party SDKs, are required to submit new apps and app updates to the App Store.

  • Apple App Store

App Review requires final builds with working URLs, and demo account details (with the back end switched on) if the app has a login; incomplete bundles and builds that crash are rejected (Guideline 2.1).

    • Confirm the developer account is a legal entity if you provide healthcare services
  2. Week 2 · days 8–14

    Store optimization

    • Research store keywords for the needs you address (e.g. anxiety tools, mood tracking)

  3. Week 3 · days 15–21

    Clinician channel and content

    • Publish a page for therapists with evidence summaries and a free professional account

    • Reach out personally to a handful of clinicians who already use digital tools
  4. Week 4 · days 22–30

    Careful reach

06 · AVOID

Common mistakes

  1. Sharing health data with ad platforms

The FTC's 2023 BetterHelp order banned sharing health data for advertising and required refunds. Apple's guideline 5.1.3 also bars using health data for advertising. Keep pixels off sensitive flows.

  2. Treatment and cure claims

'Treats depression' is a medical claim. Apple scrutinizes medical apps and asks for regulatory documentation; describe the techniques you offer and remind users to consult a professional.

  3. Pitching in support communities

Dropping your app into someone's crisis thread is harmful and gets you banned. Point to crisis resources and earn mentions through usefulness elsewhere.

  4. Submitting a healthcare-service app as an individual

Apple's 5.1.1(ix) expects apps in highly regulated fields like healthcare to come from a legal entity that provides the service. Sort this out before launch week.

07 · WHAT OUTPOST DOES

How Outpost helps

  • Checks every claim in what it drafts against your confirmed facts, so no medical or cure claim goes out

  • Drafts a readable privacy page from the facts you confirm about your data handling

  • Writes skill articles for clinician review, then tracks their rankings over the following months

  • Drafts a clinician-facing article and personal outreach to therapists for your approval

  • Writes articles for awareness dates, which you place on the Calendar, and logs every post with a receipt

Every action Outpost takes gets a receipt: the live URL, a timestamp and what it drove, or “nothing yet”.See how receipts work

08 · QUESTIONS

Frequently asked questions

Can we say our app treats anxiety or depression?

Only with the regulatory backing to support it. Apple reviews medical apps with greater scrutiny and asks for documentation of clearance; most wellness apps describe techniques and benefits instead and remind users to consult a professional.

Is it OK to use Meta or TikTok pixels?

Keep them off anything that reveals health information. The FTC's BetterHelp case was about sharing health questionnaire data with advertisers, and Apple's rules bar using health data for advertising. Outpost's plans work at $0 ad spend anyway.

How do we reach therapists?

Make it easy for them: evidence summaries, a free professional account, handouts and clear privacy. Then reach out personally to clinicians who already recommend digital tools.

Should we post in mental health subreddits?

Generally no. Support communities exist for support, and most restrict promotion. Contribute only where rules allow and never in a crisis thread.

Do awareness months actually help?

They raise attention, but people notice opportunistic campaigns. A genuinely useful contribution (a free resource, a clinician Q&A) fits better than a discount.

How long does this take?

Store optimization can move within weeks; content and clinician referrals compound over months. Trust is built slowly in this category.

Which plan fits?

Growth ($149) for most apps; Runway ($49) if you are pre-launch and setting up listings and the privacy story. All plans work at $0 extra spend.
