AppsPeriod and fertility tracking apps playbook

Market your cycle tracker on what users check first: privacy

Since the FTC's Flo Health and Premom cases, people ask where their cycle data goes before they ask about features. Outpost builds your campaign around clear privacy answers, life-stage content and the communities where people compare trackers.

01 · DISCOVERY

How customers find you

Users find period and fertility apps through store search, friends, clinicians, and communities organized around a life stage or condition: trying to conceive, PCOS, endometriosis, perimenopause. Privacy became a public buying criterion after the FTC's 2021 order against Flo Health and its 2023 case against Premom, both over sharing health data with analytics and advertising companies. Every life stage is a different audience with different questions.

The buying journey

Download, log a cycle or two, and judge whether the predictions feel right. Paid tiers usually unlock fertility-window insights, condition tracking or pregnancy modes. Trust builds slowly and breaks fast: a clear data policy, no ad SDKs and visible data controls are reasons to switch from an incumbent.

What app makers aim for

  • Become the trusted choice for people who prioritize privacy
  • Win life-stage searches (trying to conceive, PCOS, perimenopause) with dedicated store pages
  • Earn recommendations from clinicians, educators and condition communities
  • Publish health content careful enough to be trusted
  • Be ready with clear answers when privacy news sends people looking for alternatives

02 · CHANNELS

Channels, ranked by what matters most

Ranked 5 (essential) to 1 (optional) by the research for this industry.

  1. App Store and Google Play with life-stage product pages

    Essential

Store search is the main entry point. Custom product pages let 'ovulation tracker', 'PCOS tracker' and 'perimenopause tracker' searchers each land on screenshots made for them, with their own keywords.

  2. Privacy transparency content

    Essential

A plain-language 'where your data goes' page with an SDK list and data controls answers the question that now decides installs. If you're covered by Washington's My Health My Data Act, the state also expects a separate consumer health data privacy policy linked from your homepage.

  3. Life-stage and condition communities

    High

Trying-to-conceive, PCOS, endometriosis and perimenopause communities discuss trackers in depth and have strict rules. Honest, disclosed participation works. Drive-by promotion doesn't.

  4. Careful health-education SEO

    High

Questions about cycle phases, ovulation signs and symptoms are steady search demand. This is health content: cite medical sources, use qualified reviewers, and never imply diagnosis.

  5. Clinicians and health educators

    Medium

Clinicians, fertility educators and sex-ed creators recommend tools to patients and audiences. A one-page clinician summary of what the app does and doesn't do helps them say yes.

How you make money changes the plan

The same channel can be a strong fit for a paid game and a poor one for a free one. Each line below is a rule or rating from Outpost's knowledge base, with its source; your own plan is built from your answers.

Free with in-app purchases

  • Poor fitF-Droid

F-Droid accepts only free/libre open-source apps, strictly forbids proprietary tracking or advertising libraries (e.g. Firebase, Play Services), and flags remaining ads, tracking or non-free network dependencies as anti-features.

Free with ads

  • WorksApple App Store

Apple rejects apps designed predominantly to display ads, limits display ads to the main app binary, requires interstitials to be labelled as ads with easy close/skip buttons and a way to report inappropriate ads, and bars third-party ads and analytics in Kids Category apps (except limited contextual ads from services that human-review creatives).

  • Poor fitF-Droid

F-Droid accepts only free/libre open-source apps, strictly forbids proprietary tracking or advertising libraries (e.g. Firebase, Play Services), and flags remaining ads, tracking or non-free network dependencies as anti-features.

  • WorksGoogle Play

Google Play bans unexpected full-screen interstitials, full-screen interstitials that cannot be closed after 15 seconds (opt-in or non-interrupting ones, such as after a score screen, may run longer), ads that mimic app or system UI, and lock-screen monetization (unless the app is a lock screen); rewarded ads the user opts into are exempt.

Subscription

  • WorksApple App Store

Apple auto-renewable subscriptions must provide ongoing value, last at least seven days, work across all of the user's devices, and clearly describe what the user gets before asking them to subscribe.

  • Poor fitF-Droid

F-Droid accepts only free/libre open-source apps, strictly forbids proprietary tracking or advertising libraries (e.g. Firebase, Play Services), and flags remaining ads, tracking or non-free network dependencies as anti-features.

Freemium

  • Strong fitAlternativeTo

AlternativeTo classifies apps as Free, Freemium (free version can 'perform the main purpose of the app'), Free for personal use or Commercial (time- or usage-limited versions count as Commercial), offers an 'Only Open Source' filter, rejects Coming Soon, closed/private beta, invite-only and Early Access apps, and will probably accept open betas.

Paid random items (gacha, loot boxes)

  • Apple App Store

On the App Store and Mac App Store, unlocking features, subscriptions, in-game currency, premium content or a full version must use Apple In-App Purchase, and apps may not use their own unlock mechanisms such as license keys.

  • Apple App StoreSome markets only

Since iOS 26.5 (terms published 18 June 2026), Brazilian iOS apps may use alternative in-app payments or link out to the web: App Store commission is 21% (10% for Small Business Program members and for subscriptions after year one) plus 5% if Apple processes the payment, link-out sales within 7 days carry a 15% (or 10%) store-services commission, and alternative marketplaces carry a 5% Core Technology Commission.

  • Google PlaySome markets only

Google Play user choice billing (an alternative billing system offered alongside Google Play billing, with a 4% service-fee reduction on those transactions) is available in India, and in Indonesia and Brazil under the pilot.

  • Apple App StoreSome markets only

Since Japan's Mobile Software Competition Act took effect in December 2025 (iOS 26.2), iOS apps in Japan can be distributed through Apple-authorised alternative marketplaces and can offer alternative payment processing or web purchase links alongside Apple IAP; App Store commission is 10% or 21% plus 5% for Apple IAP, web-link purchases pay 15% (10% for reduced programs), and apps outside the App Store pay a 5% Core Technology Commission.

Technical eligibility

  • Apple App Store

Apple moved to a new age-rating system; developers had to answer the updated age-rating questions by 2026-01-31 to avoid an interruption when submitting app updates.

  • Apple App Store

An App Clip's uncompressed, thinned main bundle may be at most 15 MB for iOS 16 and 10 MB below iOS 16; 100 MB is allowed only for iOS 17+, digital-invocation-only clips used where reliable internet is likely.

  • Apple App Store

App privacy details (the 'nutrition label'), including the data practices of third-party SDKs, are required to submit new apps and app updates to the App Store.

  • Apple App Store

App Review requires final builds with working URLs, and demo account details (with the back end switched on) if the app has a login; incomplete bundles and builds that crash are rejected (Guideline 2.1).

06 · AVOID

Common mistakes

  1. Privacy promises your SDKs don't keep

The FTC alleged that Flo promised privacy while sending health events, such as pregnancy, to analytics and ad providers. The settlement required user consent before sharing and an independent privacy review. Your SDK list has to match your policy.

  2. Ad pixels on health pages

Retargeting tags on condition or fertility content can send health inferences to third parties. Under the FTC's amended Health Breach Notification Rule and state laws, that's a legal risk as well as a trust risk.

  3. Implying contraception or diagnosis

Positioning a tracker as birth control or a diagnostic tool moves you into medical-device territory. Apple asks medical apps to disclose their methodology and to remind users to check with a doctor.

  4. One message for every life stage

Someone trying to conceive, a teen with a first cycle and someone in perimenopause want different things. A single generic pitch converts all of them poorly.

  5. Casual health content

Unsourced health articles erode trust and tend to perform badly. Cite sources, use qualified reviewers, and say plainly what the app can't tell users.

07 · WHAT OUTPOST DOES

How Outpost helps

  • Turns your answers about data handling into a plain-language privacy page.

  • Proposes life-stage keyword sets and screenshot briefs for your listings.

  • Drafts health-education articles you can hold for expert review before they publish, and logs every publish with a receipt.

  • Measures without ad pixels: Search Console, the numbers you share and tracked links only.

  • Tracks whether AI assistants name you for prompts like 'most private period tracker app'.

Every action Outpost takes gets a receipt: the live URL, a timestamp and what it drove, or “nothing yet”.See how receipts work

08 · QUESTIONS

Frequently asked questions

Why lead with privacy instead of features?

Because users now ask about it first. The FTC's Flo Health order in 2021 and its 2023 Premom case were both about sharing sensitive health data with third parties. A clear, true privacy story is a competitive advantage, as long as your SDKs actually match it.

Does HIPAA apply to a consumer period tracker?

Usually not, but that doesn't mean no rules apply. The FTC's 2024 Health Breach Notification Rule amendments cover health apps outside HIPAA, and states such as Washington have consumer health data laws. Ask counsel about your situation; Outpost flags questions but doesn't give legal advice.

Can we market the app as natural birth control?

Not without the regulatory clearance that claim requires. Apple also reviews medical claims more closely and asks for methodology. Market tracking and insight, not contraception or diagnosis.

How do we measure marketing without ad pixels?

With Search Console, App Store and Play Console analytics, and UTM-tagged links. That's enough to see which pages and communities bring installs, and it keeps third-party tags off your health pages.

Are there femtech directories worth submitting to?

Few that bring real users. The stores, AlternativeTo and well-moderated condition communities matter more. If your app is open source and tracker-free, F-Droid is worth it.

How long until this shows results?

Store page changes can move within weeks. Health content and community trust build over months, and they're the assets that hold when a competitor has a privacy scandal.
